Registering with the practice

PreRegistrationYou can bring in your ID as listed below and complete the registration forms at the practice, or:

Please print off a registration form and new patient registration questionnaire, fill them out and bring them in with the required ID.

Over 16s and adults: photo ID (passport or driving licence) and proof of address (bank statement, utility bill or tenancy agreement) and hand it in to the practice reception to start your registration process. You will be contacted to come in for a new patient health check. After this, you can book an appointment with a doctor

5-16 year olds: passport or full birth certificate

Under 5s: full birth certificate and red book - an extra form will also need to be completed with regards to Childhood Immunisations.

Please ensure you have your NHS number as this will speed up us receiving

your records from your previous GP.

Registering for online services

You can also register for Patient Access, which lets you use our online services. If you want to register for this, ask at reception or print and fill in the following form and hand it to reception along with both a form of photo ID (for example, your passport) and proof of address (for example, a utility bill).

The practice currently has a patient participation group (PPG) who we actively consult with about our ideas for developing the practice and our services, and from whom we can gather further feedback and suggestions.
